The Office of Care Management at Dartmouth Hitchcock Medical Center (DHMC) has career opportunities for Case Managers. Qualified candidates will possess both recent acute hospital care experience and Case Management experience. DHMC Case Managers manage complex discharge plans, while collaborating with a team of nurses, social workers and providers on the patient’s behalf. These positions require excellent communication skills with the ability to educate family members and patients on their discharge plans, engage in difficult conversations around patient care and have the network of support services secured prior to discharge. Qualified candidates will have well-developed critical thinking skills and be able to develop parallel discharge plans to be prepared for the unexpected.
