Capacity Coordination Center Nursing
The Capacity Coordination Center is a service available 24 hours a day, seven days a week to help facilitate urgent/emergent transfers to Dartmouth Hitchcock Medical Center (DHMC) and soon to include the full Dartmouth Health system. Calls to the Capacity Coordination Center are answered by an experienced Registered Nurse who collect all pertinent information while contacting the respective DHMC clinical service provider(s).
The Capacity Coordination Center for Dartmouth Health utilizes data driven decision making, coordinates information, and aligns resources to facilitate patient transfer and placement to the appropriate level of care.
Our inter-disciplinary staff uses technology including computer applications, real-time dashboards, and daily collaboration with system staff to accomplish their work.
Our Capacity Coordination Nurses and Specialists are the most visible members of the Capacity Coordination Center. They communicate bed availability to incoming patients, physicians and nursing staff to ensure the earliest possible bed placement for patients coming from outside the facility, the Emergency Department or the Operating Room.

- Obtaining and documenting relevant clinical data to individually assess the needs of each patient to ensure appropriate level of care and proper placement of patients.
- Acting as a navigator for referring providers in identifying and accessing appropriate Dartmouth Health services.
- Managing and coordinating communications between DH providers and other facilities via telephone and electronic communication methods.
- Identifying patients and facilitating redirect and placement to Dartmouth Health system member facilities as well as monitoring referrals to system members and collaborating with local decision makers to identify barriers and resources required to support system transfers.
